In general, it is not required to have a doctor’s note to receive a massage therapy treatment. However, some insurance companies may require a doctor’s note before direct billing. Please check with your insurance if you need one. A doctor’s note is only valid for a year.
Massage Therapist Preparation: Will arrive approximately 15 minutes before the start time to set up the equipment. Will bring a professional massage table with a table warmer, freshly laundered linens, towels, blanket and premium massage oil or lotion. I can accommodate you if you want me to use your bed linens, towels, blanket. Ensure the equipment is cleaned and disinfected before and after each treatment. At the end of your session, discard all used linens, towels, and blankets in a sealed plastic bag. Client Preparation: Please keep your pet and children away from the treatment area and keep the floor area clutter free to be able to move around during your MT session. Remove all jewelry and store it in a safe place to be able to massage the body area thoroughly.
Before your MT session, you will fill up the Massage Therapy Intake form and Consent and Authorization for Electronic Claim Submission and Assignment of Benefits form for direct billing. We will update your file once a year or if there are any changes in your overall health condition. Additional assessment and treatment consent forms will be discussed with you prior to your session as necessary. All information is kept completely confidential. I will perform an assessment and create a treatment plan that is safe and appropriate for you. I will incorporate a combination of techniques and modalities such as myofascial release, trigger point therapy, Swedish or deep tissue therapy, Joint Mobilization, PNF Stretching and incorporating various therapeutic modalities like medical acupuncture, hot stone therapy, reflexology, and cupping therapy. Will recommend some remedial exercises as part of your treatment and home care.
Yes, your extended health benefit may cover all or a portion of the cost of your massage therapy service and you just need to pay the remaining balance not covered by your insurance provider. Please Keep in mind some insurance companies may require a doctor’s note. Check your insurance coverage before booking an appointment. If direct billing does not work, full payment of the service is your responsibility to pay.
You can settle your massage therapy treatment after session with cash, a cheque or an interact etransfer or Visa, Master Card and Debit card. For chair massage the lead person can make payment through e-transfer and receive a consolidated e-receipt for the transaction or each recipient has the option to pay individually after a massage session.
